Abstract: Acute respiratory failure (ARF) remains the most frequent indication for ICU admission among patients with cancer. Common causes of ARF include pneumonia, chemotherapy-and radiation-induced lung damage, venous thromboembolism, diffuse alveolar hemorrhage, pulmonary leukostasis, and acute respiratory distress syndrome (ARDS). Detection of the underlying cause of ARF is often challenging in patients with cancer because disease can present atypically. For example, cough and sputum production are uncommon in patients with bacterial pneumonia. Patients with chemotherapy-induced lung injury may present with nonproductive cough and exertional dyspnea that develops over several weeks of treatment. Diffuse alveolar hemorrhage is especially common in persons with leukemia who are thrombocytopenic. Widespread destruction of alveolar epithelium and flooding of alveolar spaces is a hallmark of ARDS.
