The lateral thoracic vascularized lymph node transplant (VLNT) is an underutilized but very powerful and versatile donor site for a wide variety of lymphatic defects. It fulfills the requirements of a small buried flap when required yet contains a large number of lymph nodes. This donor site can also provide extensive skin and soft tissue coverage when including the thoracodorsal vessels to treat the most challenging defects associated with radiation and lymphedema. It is a safe and reliable option when used with reverse lymphatic mapping.
