Nd-YAG laser resection of endobronchial lesions is now a well-accepted treatment modality with a high rate of success, especially for palliation of otherwise inoperable tumors. Patient selection is of the utmost importance, and we reserve this therapy mainly for those who are symptomatic or who have a recalcitrant pneumonitis behind an obstructing lesion. Treating a lesion 'because it is there' or on the basis of radiographic findings alone subjects a patient needlessly to a small but significant risk. The use of the laser to excise benign tumors or airway strictures may, in many instances, obviate the need for operation. |
