Abstract: Fluconazole is a promising azole antifungal with in vitro and clinical activity against a wide variety of pathogenic fungi. In particular, fluconazole may have a potential place in the prophylaxis and perhaps treatment of immunocompromised patients with fungemia when the putative pathogens are either Cryptococcus neoformans or Candida spp. Based upon the apparent margin of safety, long-term suppressive therapy is perhaps one of the most interesting aspects to the use of fluconazole in those patients previously treated with amphotericin B. Although treatment of infection resistant to other antifungals (including amphotericin B) has been suggested, the lack of fluconazole's fungicidal effect will limit its usefulness in this setting. However, fluconazole's extensive penetration into the cerebrospinal fluid and high therapeutic index make it a welcome addition to the antifungal armamentarium.
