Abstract Title: Pulmonary function tests post medialization for unilateral vocal cord paralysis
Meeting Title: CHEST 1996
Abstract: Purpose: To evaluate changes in pre and post operative pulmonary function tests and O2 % saturation in patients undergoing silastic cord medialization and to subjectively evaluate the quality of voice and cough. Methods: Patients with intrathoracic malignancies and unilateral vocal cord paralysis who underwent silastic vocal cord medialization were evaluated. Pulmonary function tests (PFTs) pre and post vocal cord medialization were performed. The variables evaluated were FEV1 (%pred), FEF50/ FIF50, FEFmax (%pred) and PIFR (L/S). The Flow Volume Loops (FVL) and O2% saturation were compared. A quality of voice and cough questionnaire was answered by the patients. Means were compared with t-test for paired samples. All data are presented as means±SD. Results: Ten consecutive patients requiring medialization were entered into the study. No statistically significant changes occurred in the parameters evaluated. Thirty percent of patients developed new FVL abnormalities after medialization consistent with either a variable or fixed extra thoracic upper airway obstruction (UAO). All patients had subjective improvement in their quality of voice and cough. FEV1 FEF50/ FEFmax (%pred) FIF50 (%pred) PIFR (L/S) PRE 65±24 83±65 55±16 2.92±0.98 POST 64±32 87±60 48±16 2.46±0.80 Conclusion: There is no significant change in PFTs after silastic vocal cord medialization. All of the patients had a subjective improvement in their quality of voice and cough. Clinical Impression: Successful amelioration of symptoms in all of the cases coupled with limited morbidity justifies treatment of this condition.
