Paraneoplastic syndrome refers to clinical manifestations of renal disease which cannot be attributed to direct tumor burden, infiltration, or metastasis, but are instead caused by secretion of hormones, growth factors, cytokines and tumor antigens from the tumor itself. A spectrum of glomerular histopathology has been described in association with a variety of primary neoplasms and a variety of renal functional and structural abnormalities have been described in patients with neoplastic disease. The objectives of this chapter are to (1) describe the histopathologic features of primary GNs; (2) report on the prevalence of PNGNs and their associated malignancies; (3) discuss the purported pathogenetic mechanisms for neoplasia-induced glomerular injury based on information obtained from case reports, clinical and experimental models of PNGN; and (4) offer suggestions for the screening and management of patients with suspected PNGN. © 2014 Elsevier Inc.
