Article Title: | Current management of rectal cancer: Total mesorectal excision (nerve sparing) technique and clinical outcome |
Abstract: | The clinical results initially reported for TME-based operations have been reproduced in series with sufficiently large patient cohorts. The long-term follow-up indicates that the results observed at 5 years are durable and associated with cancer cure and not delayed time to recurrence. This increase in cure and local control has been accompanied by dramatic increases in sphincter, sexual, and urinary function preservation. TME-based operations are now established as the standard of care for rectal cancer and form the basis for trials investigating the role of adjuvant therapy. Educational efforts are currently focused on spreading the technique of TME-ANP, and improvements in technology are leading TME into the era of minimally invasive surgery. © 2005 Elsevier Inc.
