Purpose of Review: This review summarizes the recent literature surrounding anesthetic options for same-day surgery total joint arthroplasty. It details the recommendations for preoperative, intraoperative, and postoperative anesthesia care to optimize the total arthroplasty patient for ambulatory surgery. Recent Findings: There is a greater impetus to discharge patients on the same day for total joint arthroplasty. Selection of appropriate patients is critical to ensure safety and success. Enhanced recovery protocols are implemented to assist with patient optimization. The anesthesiologist can contribute to successful discharge by utilizing multimodal analgesia (including regional techniques), employing short acting spinal anesthesia where appropriate, implementing appropriate fluid management strategies, prophylactically treating for post operative nausea and vomiting, and administering tranexamic acid to decrease blood loss. Summary: Anesthetic choices contribute enormously to the successful same-day discharge of the total arthroplasty patient. Major barriers to same day discharge include orthostatic hypotension, nausea and vomiting, and post operative urinary retention, signaling areas of future improvement and research opportunities. © The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature 2024. |
