Whole slide imaging is revolutionizing the field of pathology and is currently being adopted for clinical, educational, and research initiatives by an increasing number of institutions. Pathology departments have distinct needs for digital pathology systems, yet the cost of digital pathology infrastructure and operation is cited as a major barrier for widespread adoption by many organizations. Many of the costs that are associated with digital pathology are specific to use cases, size of operations, geographical location, and the extend of the implementation. This chapter describes the tangible and intangible benefits, and the business case for digital pathology. It includes the required investments and operating cost considerations, as well as potential revenue and cost savings that are reported with digital pathology operation. In addition, the chapter includes a discussion of the metrics that should be used for calculations for the development of custom digital pathology business plans. © 2025 Elsevier Inc.
