Abstract: Reduced kidney function is a frequently encountered complication following hematopoietic stem cell transplantation (HSCT). The spectrum of kidney complications following HSCT ranges from mild elevations in serum creatinine and proteinuria to clinically significant acute kidney injury (AKI) requiring dialysis and chronic kidney disease including end-stage renal disease. An understanding of renal pathophysiology will help identify the patients at high risk for kidney failure and prevent or reduce the severity of kidney dysfunction post HSCT. The kidney injury occurs via multiple pathologic mechanisms resulting in definite patterns of injury including thrombotic microangiopathy (TMA), tubulointerstitial nephritis and podocytopathy. Proteinuria is common after HSCT and occurs more frequently than TMA. Persistent high-level BK virus replication is the hallmark of BK nephropathy in kidney transplant recipients and of hemorrhagic cystitis in post HSCT patients. The approach to preventing and mitigating kidney injury post HSCT should begin with the assessment of baseline kidney function at the time of evaluation. © 2021 John Wiley and Sons Ltd.
