Abstract: Staging in thyroid cancer is used to provide an estimate of risk of death and recurrence (initially and on follow-up), with the goal of tailoring diagnostic and therapeutic strategies for individual patients. Traditionally, these staging systems were (1) designed to predict risk of death, (2) applied in the initial visit, (3) static, and (4) included a defined set of clinical factors and pathologic characteristics like age at diagnosis, size and histology of the tumor, completeness of resection, and presence of distant metastases. More recently, it has become clear that a comprehensive initial risk stratification requires review of all available data that is obtained up to 4 months of initial diagnosis and treatment. The current American Thyroid Association (ATA) guidelines recommend the initial combination of traditional staging systems that better predict disease-specific survival with a relatively new ATA risk stratification system that is optimized to predict tumor recurrence. During follow-up, this staging exercise becomes a dynamic process that incorporates clinical data as it becomes available, and it intends to capture the response to therapy of individual patients as well as the behavior of the tumor. In the following chapter we describe the current way of combining traditional staging systems with the ATA Risk Stratification system to better predict survival and recurrence initially, and we highlight the importance of using a dynamic staging system during follow-up that incorporates clinical and diagnostic information (as it becomes available) and better captures the individual response to therapy of patients as well as the behavior of the tumor. A continued and dynamic risk stratification ensures that the intensity of diagnostic and follow-up strategies will be tailored to the individual risk of recurrence and death of each patient. © 2021 Elsevier Inc.
