We have combined the advantages of computerised tomography (CT) localisation and biplane fluoroscopy to perform outpatient percutaneous implantations of Iodine 121 seeds for the treatment of unresectable or recurrent tumours of the pelvis using a transperineal approach. The technique is based on the principles of CT-guided percutaneous biopsy. Tumours up to 16cm as measured from the perineum have been amenable to this procedure. Twelve cases have been performed using this technique. Types of tumour treated include 5 cases of recurrent carcinoma of the rectum, 5 cases of unresectable carcinoma of the prostate Stage II-B or above, I case of adenocarcinoma of unknown primary origin involving the pelvic wall and 1 case of recurrent sarcoma of the rectum. Local control was achieved in 8 of 12 patients; local control was also achieved in 2 other patients, but they developed distal metastasis. The last 2 patients developed local recurrence. A small fistulous tract developed in 1 patient, no other complications were encountered. © 1989. |
