Abstract: Graft failure (GF) following an allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a complex syndrome characterized by pancytopenia and hypocellular or acellular bone marrow. Graft failure can be defined based on the mechanism leading to it or the timing of the event. It was one of the major causes of treatment failure in the early era of allo-HSCT, however, now a days, this complication is less prevalent, but when occurs remains a devastating complication with very poor outcomes. Recognizing the patients who are at highest risk for developing graft failure and interventions to reduce this risk prior to transplant are of critical importance. Early diagnosis and ability to identify the mechanism of graft failure once occurred are important in order to provide the right intervention and reduce complications such as infections, bleeding, as well as complications of massive transfusions such as iron overload. © 2019 Elsevier Inc.
