Abstract: Vulvar cancer is a rare malignancy of the female genital tract. Among other prognostic factors, numerous studies have highlighted that inguinofemoral lymph node status is the most important determinant of survival. Therefore, evaluation of the groins for possible metastases is an integral part of the surgical staging for these patients. Clinicopathologic studies have demonstrated which patients may have the highest risk for inguinofemoral node spread. The high morbidity rates associated with full inguinofemoral lymphadenectomy (IFLD) led researchers to revise the indications of the procedure and develop novel techniques of inguinofemoral lymph node evaluation. Two large, prospective trials confirmed that sentinel lymph node (SLN) biopsy is an accurate method to assess groin node status without compromising oncologic outcomes. Importantly, the SLN approach should be used in patients who meet the strictly defined criteria and performed by experienced surgeons. Future research will further clarify the optimal management of patients with positive SLNs. © Springer International Publishing AG 2018.
