Abstract: Historically, a diagnosis of breast cancer required radical mastectomy, resulting in profound disfigurement. Today, with advances in multidisciplinary oncologic therapy and reconstructive techniques, most women are candidates for immediate reconstruction at the time of skin-sparing mastectomy. In this chapter, we address the controversies in breast reconstruction, including the timing of reconstruction for patients who will require postmastectomy radiotherapy. We also review the data on the oncologic safety and cosmetic results of nipple-sparing mastectomy. © Springer-Verlag Italia 2013.
