Abstract: In the Clinical Dilemma slot the editors pose a question to an acknowledged expert in the field. In this issue we address the problem of a sarcoma in the posterior aspect of the thigh. At operation, despite preoperative magnetic resonance imaging, the lesion is unexpectedly found to involve the sciatic nerve. How should one proceed? Professor Murray Brennan of New York replies.
