Abstract: (create) This chapter explores secondary premature ejaculation (PE). It begins by discussing physiology and pathophysiology of erectile function. It then examines the central, autonomic and somatic nervous systems participation in the regulation of penile erections. The chapter goes on to consider vascular, endocrine, and cavernosal smooth muscle factors. Next, molecular mechanisms are described. It then looks at the role of adrenaline in detumescence. The chapter then focuses on topics of PE including etiology of PE, prevalence of secondary PE, and studies of secondary PE management. The chapter closes by discussing the clinical care pathway.
