Abstract: Surgical resection of intrahepatic cholangiocarcinoma is the only potentially curative treatment option. However, due to the natural history of the disease, most patients present at an advanced stage at which liver resection cannot be considered. As there has been little progress in the development of efficacious chemotherapy and other non-surgical treatments, patients with potentially resectable tumors are generally offered a surgical approach that should include removal of lymph nodes in the gastroduodenal ligament for adequate staging assessment despite no known impact on survival or local recurrence. The aim of this chapter is to present the actual indications for resection, the outcomes after surgery including assessment of prognostic factors and to discuss the role of reresection, liver transplantation and implant of intraarterial pump for hepatic arterial infusion in unresectable ICC. © 2020 by Nova Science Publishers, Inc.
