Article Title: Optimizing lung cancer radiotherapy treatments using personalized dose-response curves
Abstract: In this chapter we review the use of prediction models to personalize the physical parameters associated with lung cancer radiotherapy treatment planning, including the fractionation schedule. New models and data are continuing to emerge that support moving away from a one-size-fits-all, or one-size-fits-many approach. In most cases, radiotherapy for non-small lung cancer is a toxicity-limited treatment with still suboptimal tumor local control. We will discuss the dose-volume determinants, for important lung cancer radiotherapy endpoints, including esophagitis, pneumonitis, bronchial stenosis, and cardiotoxicity. We will discuss in detail tumor response and tumor local control modeling, and how to design a clinical trial to use personalized toxicity and tumor control curves to individualize dose and fractionation. The main point of this chapter is that a new clinical paradigm is emerging of using published and validated models to inform treatment decisions on a patient-specific basis. © 2022, The Author(s), under exclusive license to Springer Nature Switzerland AG.
