This chapter will provide an overview of the various types of benign and malignant papillary lesions as well as epithelial–myoepithelial lesions with a focus on diagnosis in a core biopsy sample. Traditionally, the standard practice has been to excise any papillary or epithelial–myoepithelial lesions. In recent years, data have shown low upgrade rates on excision when benign papillomas are diagnosed in a core biopsy, and there has been a trend to not excising papillomas. A conservative approach to interpreting core biopsies containing atypical papillary lesions and papillary carcinomas is prudent. This is particularly true in assessing the presence or absence of an invasive component in a papillary carcinoma, which is usually easier when the entire lesion can be evaluated.
