Postmastectomy radiation is an integral part of breast cancer treatment. There are clear guidelines for administration in advanced disease. However, its use in early-stage breast cancer and in the setting of neoadjuvant systemic therapy remains an area of controversy. Recent consensus guidelines by the American Society of Clinical Oncology, American Society for Radiation Oncology, and Society of Surgical Oncology aim to shed light on these areas of controversy.
