Article Title: | Quimioterapia a altas dosis en el cáncer de mama metastásico: Múltiples ciclos y expansión ex vivo de células progenitoras hematopoyéticas |
Abstract: | Purpose: To update 1) the strategy of multiple cycles of high-dose chemotherapy (HDC) in metastasic breast cancer and 2) recent advances in the hematological support of HDC. Material and methods: Review of the basis and results of multiple cycles of HDC and historical comparison with a single cycle of HDC. Review of state-of-the-art techniques for hematological support, including ex-vivo expansion and CD34+ selection of hematopoietic cells. Results: Preclinical models support the research of multiple cycles of HDC. Studies conducted at Memorial Sloan-Kettering Cancer Center establish the feasibility of this strategy. Preliminary data show equal of higher antitumoral activity than a single cycle of HDC. CD34 selection and ex-vivo expansion of progenitor cells yield a rapid hematological recovery, may widen the number of patients candidates to HDC and may facilitate the delivery of multiple cycles of HDC. Conclusions: Randomized trials may be needed to compare multiple cycles versus a single cycle of HDC. CD34+ selection and ex-vivo expansion of progenitor blood cells will facilitate multiple-cycle HDC protocols. |
