Thoracoscopy is not a new procedure, but with the improved equipment now available, its use has been extended beyond simple diagnostic and therapeutic interventions of the pleural space. The potential benefits of video-assisted thoracoscopy include: improved visualization, the ability to remove portions of lung, decreased post-operative pain, improved cosmesis, increased patient acceptance, a shortened hospital stay and decreased cost. There have been studies suggesting a lessened immunosuppressive effect of less invasive procedures. Before applying this technique for the management of lung cancer, surgeons must always be cognizant of the oncologic principles that have been developed and the lessons we have learned over the past 70 or more years in managing lung cancer. Video-assisted thoracoscopic techniques have now been employed in the diagnosis, staging and therapy of lung cancer. |
