Abstract: The current management of rectal cancer is multifaceted with both operative and nonoperative treatment modalities available to the surgeon and the patient. The selection of the optimum therapy requires accurate preoperative assessment of all tumor-related and patient-related factors. This preoperative assessment is critical to the selection of the optimum treatment for a patient with rectal cancer. Endorectal ultrasonography has emerged as the best modality for the accurate assessment of depth of rectal wall invasion and lymph node status and can direct patients to receive local therapy for early confined lesions or preoperative adjuvant therapy followed by resection for locally advanced lesions. Hence in 1998, all rectal cancers should be staged with rectal ultrasonography before operative intervention.
